Smilelign Case Selection

Clear aligners are a very effective orthodontic treatment for many cases. There are however cases that can be difficult to treat, being too complex for clear braces that may require fixed appliances from a specialist orthodontist. Here are some guidelines to help with case selection. These are only guidelines and our Smilelign technicians will be able to advise you further by sending impressions for our 'Scan & Plan' service. We always aim to provide treatment plans that are realistic, predictable, completed within a reasonable time and do not require any specialist skills from the GDP.

We can treat cases with up to 3mm crowding per arch. Cases with up to 6mm of crowding per arch will almost certainly require Interproximal Reduction (IPR) of the enamel, we would recommend this only if the GDP is comfortable undertaking IPR. Cases with over 6mm of crowding per arch would not normally be suitable for Smilelign clear aligner treatment.

We can treat  cases with up to 3mm spacing per arch. Cases with up to 6mm of spacing per arch may require some restorative additions or leave residual spaces as part of the treatment plan.  Cases with over 6mm of spacing per arch would not normally be suitable for Smilelign clear aligner treatment.

Overjets of up to 2mm are usually straight forward to treat with Smilelign. Overjets of up to 4mm become more difficult and a compromise of a small overjet may be required. Cases with an overjet of 4mm or more would not normally be suitable for Smilelign clear aligner treatment.

Overbites of up to 1mm are usually straight forward to treat with Smilelign. Overbites of up to 3mm become more difficult as intruding teeth with clear aligners is not always simple. Cases with an overbite of 3mm or more may require bite opening appliances before using Smilelign clear aligner treatment.

Openbites can be difficult to treat with clear aligners. Small closures of up to 0.5mm per arch can be achieved. Openbites of 1mm or more may require other treatment before using Smilelign clear aligners.

Cases with up to 2mm midline discrepancies can be corrected quite easily. Cases with up to 4mm of midline discrepancies will almost certainly require Interproximal Reduction (IPR) of the enamel, we would recommend this only if the GDP is comfortable undertaking IPR. Cases with over 4mm of midline discrepancies would not normally be suitable for Smilelign clear aligner treatment.


Does Smilelign Offer A Guarantee?
Smilelign is an effective way of straightening teeth and works similar to traditional methods of orthodontics but largely depends upon a patient’s biology and compliance with treatment. As such, Smilelign as with any orthodontic treatment, cannot guarantee that a patient’s teeth will move exactly as predicted. The prescribing orthodontist or dentist is responsible to use their experience and skill to treatment plan and monitor the appropriate treatment as it progresses. If necessary, additional treatment with Smilelign or other forms may be needed. It is essential that appropriate consent is discussed and obtained from the patient prior to starting treatment.
